Verdict: Core epidemiology claims are confirmed. The only hard correction is CoNS blood wording (62% of positives, not 40%). Several resistance claims become more severe when restated as among-tested rates.

How to read these numbers (why “of all” ≠ full truth)

Reason: Hospital AST panels differ by organism, specimen, and lab workflow. Ranking “top sensitive” drugs on of-all % can over-rank agents that are simply tested more often on salvage/MDR workups (e.g. tigecycline).

Why CoNS dominate blood: Coagulase-negative staphylococci live on skin; imperfect skin prep, single-bottle draws, or colonized central lines produce this signature. That is why CoNS can be 62% of positives while major GNB are only ~19% of positives — different clinical meaning, not “more dangerous bugs.”

Why E. coli looks “more treatable” than Kleb for UTI: Among-tested gentamicin S is 85.7% (E. coli) vs 40.9% (Kleb); nitrofurantoin S 74% vs Kleb among-tested nitro only 18% S / 41% R / 42% I. Different species, different intrinsic + acquired resistance.
Caution: S. aureus vancomycin among-tested R 46% (n=13 only) is INCONCLUSIVE for true vancomycin-resistant S. aureus — too few tests; likely panel/data oddity. Do not treat as a confirmed VISA/VRSA outbreak from this alone.

Finding 4 — Comparative discoveries (with reasons)

  1. ICU vs ward Kleb hardness (confirmed): Free chest ICU Ceftaz R 77.4% (n=133) vs chest ward 61.6% (n=73). Why: ICU patients receive more prior antibiotics and invasive care; resistant clones enrich. Ward still bad, but less extreme.
  2. Cardiac–thoracic Kleb is the worst large face: pus niche; Ceftaz R 78.7%, Levo R 61.7%, Ceftriaxone R 59.6%. Why: Deep surgical infection + hospital GNB, not community UTI clones.
  3. Liver/GI Kleb different niche: urine-top sample; Meropenem appears higher on of-all S list (~32% of that unit’s Kleb). Why: Different specimen mix and possibly different clone/testing frequency — not proof liver Kleb is “fully carbapenem-sensitive hospital-wide.”
  4. Pediatric nutrition unit: Fosfomycin ranks high for urine Kleb (~42% of-all in that small subset). Why: UTI-directed agents are tested/used more in urine-heavy units.
  5. Pediatric general ward Kleb milder Ceftaz signal: of-all Ceftaz R only 26.7% (n=15) vs ICU 70%+. added Why: Small n, but directionally consistent with less ICU antibiotic pressure. Still high FQ R (Levo 66.7% of-all in that subset).
  6. FQ collapse is cross-cutting (confirmed): urine, sputum, blood all list Levo/Cipro among top resistances. Why: Years of easy oral FQ use select plasmid and chromosomal resistance across Enterobacterales and non-fermenters.
  7. Tigecycline “wins” dashboards because other drugs fail more often — stewardship warning, not preferred workhorse. Among-tested Kleb tige S 69% with 22.5% I — not perfect.

Finding 5 — Additional discoveries (new in v2)

CVP tip cultures (n=34)

Kleb 38% + S. haemolyticus 24% + A. baumannii 15% + Candida. Bridges device infection and ICU GNB. Ceftazidime resistance appears on 53% of tip rows.

CSF (n=3 only)

All three K. pneumoniae; Ceftazidime R on all. n too small for rates — flag for clinical follow-up only.

Proteus mirabilis (n=58)

Often urine/unknown; Pip/Tazo leads of-all S (34.5%); FQ and ceftazidime still frequently resistant. Intrinsically nitrofurantoin-poor — matches R nitro signal.

Burkholderia cepacia (n=17)

88% urine (unusual classic teaching = sputum/CF). AG highly R (gent 71%); Meropenem/Ceftazidime more often S among reported. Why unusual urine dominance: may reflect local sampling/coding or specific units (nutrition/nephrology) — worth lab QA check.

Enterococci

E. faecalis (n=15): Vancomycin S 100% of-all; Ampicillin S 93% — classic treatable pattern.

E. faecium (n=24): Ampicillin R 50%; Vancomycin S 75% of-all (1 R recorded). Mostly urine. Why: faecium is the ampicillin-resistant hospital species; VRE not dominant here yet.

Free vs paid GI ICU

Free GI ICU more blood-heavy (34% blood) and more S. haemolyticus; paid more urine (44%) and higher E. coli share (21% vs 11%). Kleb Ceftaz R high in both (64% free / 71% paid). Why: case-mix and sampling differ by pay-track; resistance pressure similar for Kleb.

Gram-negative dominance by adult chest clusters

Merged chest / free chest ICU / cardiac: GN ≈ 71–85% of named GP+GN microbes. Pediatric oncology closer to 50/50. Why: adult respiratory critical care selects GNB; hemato-oncology blood work recovers more CoNS.

Data-quality findings

Unknown sample type 8.6%; Not specified organism 6.6%; sterile 14.5% of all cultures. These are operational findings for lab documentation quality, not biology alone.

Finding 6 — Practical takeaways (verified logic)

Disclaimer: Secondary analytics for internal notes/education. Not a substitute for patient-level AST, infectious-diseases consult, or official hospital antimicrobial guidelines. Three-state note: small-n claims (CSF, Vanco on S. aureus) remain INCONCLUSIVE.

  1. شدة الكليبسيلا عناية مقابل عنبر: سيفتازيديم مقاومة ٧٧٫٤٪ (عناية صدر مجانى) مقابل ٦١٫٦٪ (صدر). السبب: مضادات سابقة وأجهزة أكثر في العناية تثري النسخ المقاومة.
  2. قلب وصدر أقسى وجه كبير للكليبسيلا في الصديد. السبب: عدوى جراحية عميقة لا نسخة التهاب مسالك مجتمعية.
  3. انهيار الكينولونات عام عبر بول وبلغم ودم. السبب: استخدام فموي سهل تاريخياً ينتقي مقاومة بلازميدية/كروموسومية.
  4. تيجيسيكلين «يفوز» باللوحات لأن بقية الأدوية تفشل أكثر — تحذير ترشيد لا دواء أول مريح.
